The Hawaii Water Rescue from January 25, 1993 was one of the most dangerous water rescues ever. It's the fifth escaped death in 1000 Ways to Die.

How it unfolded[]
Hugh Alexander was a tourist on Oahu, Hawaii and went missing while strong waves pulled him into a cave from which he couldn't escape the aggressive waves and exit the cave. Police officer, firefighters, paramedics and a water rescue team spent an hour looking for the missing tourist and already planned to pronounce him dead until the tourist suddenly appeared outside the cave. The water rescue team went to Hugh as fast as they could and take him back to land where he became medical treatment.
